Forms

These forms are called Notices of Arrangements and must be completed by the parents/carers of young people turning 16 or 17 and have decided not to go to school full-time. If you are staying in school full-time no forms are required.

Form A

Please complete this form if your child is undertaking a full-time course at a:

• private registered training organisation
• university/higher education institution
• Community Based Course provider

A Notice of Arrangements form is not required if your child is currently undertaking:
 

• a full-time apprenticeship/traineeship (with a completed training contract)
• a full-time TAFEWA course (with a completed TAFEWA enrolment form and signed TAFEWA parent consent form)

Form B

If your child has chosen to participate in:

• a combination of options (for example part-time schooling and part-time training).

Form C

 

If your child has chosen to participate in:

• full-time employment.
