Answers to these questions will help the young person identify their strengths, interests, needs and aspirations. They will help them understand more about themselves as a person and a learner and help them to develop a vision and goals for the future.
Young people are encouraged complete this questionnaire before the transition planning meeting. Together with information from parents, teachers and others, the young person’s responses will inform the planning process.
Some young people may require assistance in answering these questions. Symbols, photos, drawings, video/audio recording may facilitate communication.
About Me can be presented in a variety of ways including question and answer, collage, a mind map, video clip.
Some young people may have already been working on self-awareness activities such as in the Year 8 Pathway Planning and Year 9 Pathway Planning resources. These activities could inform or replace the About Me Questionnaire.
A young person completes their Personal Action Plan (or Learning Pathway Plan, as appropriate) following a transition planning team meeting and it contains goals and actions agreed on by them at the meeting. Some young people may need support to complete their plans. Personal Action Plans are reviewed and updated regularly.
MOVING ON is a planning checklist for young people with a disability, and those who support them, to consider at major transition points such as moving from school or college to the next stage of their lives. The checklist can be adapted to reflect individual needs and circumstances.
Copies can be made for school or college records and for forwarding on to post-school services and agencies.
