Fact sheets for parents and carers


Parents/carers play a vital role in their child’s the transition planning process. Particularly for young people with more complex needs, parental involvement is crucial. Research indicates that more positive post-school outcomes are achieved for young people with a disability when their parents/carers are actively involved in the transition planning process.


Together with their child, parents/carers are key members of the transition planning team.
The following fact sheets provide information for parents/carers about the transition planning process and ways they can support the process.

Questionnaires for parents and carers

The following questionnaires can assist parents/carers in putting together their views about their hopes and aspirations for their child’s future. The team coordinator can decide which questionnaire is the most appropriate. Responses to these questions can help initiate discussions at the first transition planning meeting and in developing a vision statement. Completed questionnaire can be forwarded to the transition team coordinator before the first meeting.


For parents/carers who may have difficulty with written communication, a phone conversation or meetings before the first transition planning meeting may be the most appropriate way to collect information and facilitate discussions.
