Fact sheets and templates for use by young people and educators as well as by parents and carers.
Because transition planning is individualised, flexible and responsive to individual needs and circumstances, it is important
to customise templates to reflect individual needs and circumstances. Some young people will benefit from using photos, symbols,
drawings and real objects to help them with the planning process.
