Guidelines for schools
Schools can assist effective individual transition planning by:
- engaging the student and parents/carers early to allow the student time and experiences necessary for a smooth
and successful transition to adult life
- providing students and parents/carers with accessible information about relevant post secondary pathways including
employment, further education, alternatives to employment, or a combination of these options
- developing plans in Year 8, 9 and 10 that considers post secondary pathways and builds upon student aspirations, interests,
circumstances and capabilities
- encouraging and supporting effective communication and collaboration with all agencies and services involved
- ensuring that by the age of 16, each student has an individual transition plan that identifies preferred post secondary
pathways and provides school and community-based learning experiences that are relevant to the needs of the individual.
- facilitating connections and awareness of other agencies, services, roles and responsibilities,before the student leaves
school.

for teachers are available which provide guidelines to processes and practices.
These guidelines have been developed in consultation with the West Australian Education Support Principals’ and Administrators’
Association (WAESPAA), teachers in education support schools and centres and local schools across the state and visiting teachers
from the Centre of Inclusive Schooling.
