New online materials are available to assist teachers working with students with disabilities in the transition planning process from school to adult life.

Supporting students in transition to adult life

The website includes guidelines, timelines and extensive information about pathways and transition planning within the Western Australian context including downloadable worksheets and information sheets for teachers, students and parents/carers.

The new materials aim to further enhance the educational choices, quality of service and support, and effective transition planning processes for students with disabilities.
