EDUCATION SUPPORT APPLICATIONS - STUDENT ASSESSMENT DATA FILE
The Student Assessment Data File (SADF) is currently completed as part of the application for Education Support. This has recently been updated and some changes made;
It is in line with the Department re: the requirement that parents are able to make a fully informed decision about Education Support and that they understand all appropriate options, as well as the range of Education Support options
It takes a more environmental view of the student; it has expanded reporting sections on classroom performance and behaviour, and asks for reporting on interests and aspirations
Because it is based on the Education Assistant application, it will provide schools with a familiar reporting process, it may enable a repeat reporting if a previous Education Assistant application has been completed, and it may assist the new school if an Education Assistant application is to be made.
As per the previous version, there is space for both parents and teachers to comment. This can be done in a combined meeting with the child's parent(s) and teacher(s), or separately. A copy of the Student Assessment Data File to be used is attached. This replaces a draft which was recently discussed with Student Services staff at District Office.
